PROGRAM

After many years of seeing patients put off needed dental work due to not having dental insurance, we decided to make a plan to help make dental work more affordable. We have developed our own in-office Dental Membership Program. This is unique to our office in Chatham, and there is no other plan like it! It is affordable and it has given hundreds of our patients the ability to afford the care that they need.

Our $325 annual membership includes:

2 Professional Cleanings

2 Periodic Exams

All X-rays

Oral Cancer Screening

Oral Hygiene & Homecare Instructions

2 Fluoride Treatments *Under 18*

20% Off All Other Procedures

  • The membership fee is $325 per person per year and is due in full upon joining

  • To receive the discount, all payments are due in full when services are rendered

  • It is the sole responsibility of the member to maximize their benefits within the 12 month period

    • A refund will not be given for services not used within the 12 month period

  • Renewal of the plan must be at least 12 months from the original purchase

  • NOT a dental insurance plan and can only be used in our office

  • Non-transferable and non-refundable

  • Cannot be combined with any other insurance coverage or discount program

  • Discounts do not apply to any prior treatments or procedures

  • Referrals to specialists are not covered by your membership program

  • Discounts do not apply to medications or products dispensed 

  • Renewal fee is subject to change

  • Patients with gum disease may need additional treatment at 20% off
